Submit a Care List Request

Use this form to submit information to the Pattonville Human Resources Center regarding the death or serious illness of a current or former employee or the birth of a child of a current employee. This information is shared each week with our staff.

 

Certified Staff Change of Status Forms

(Change of Status/Request for Leave of Absence) 

 

This form is needed in conjunction with Pattonville School District Policies GCBC-R-2 and GCBD-R-3. Please submit a change of status form based on the school where you're FTE is primarily located for the following leave categories: bereavement and critical illness, family leave, legal leave, military leave, religious leave, and dock leave. This form does not need to be submitted for certified staff sick leave.

Coursework and Salary Advancements

Forms are used in conjunction with coursework approval and tuition requirements in policy GCBA-R-1:

• Graduate Coursework (Graduate Coursework Approval for Advancement on the Salary Schedule)

• Application for Tuition Reimbursement for Certified Staff

 

Change in Teaching Assignment

Policy GCI-R states, “An employee who desires a change in teaching assignment within his/her own building should complete a consideration for change in teaching assignment form for the upcoming school year by the first Friday in January.”

If ever, a teacher or any staff member of the Pattonville School District is injured or is in an accident during work hours, please click on the link below that pertains to you and complete an Injury Incident Report.

Supervisor Accident Investigation Report

Steps to Take if Injured on the Job:

1. Immediately report injury to your supervisor and/or school nurse.
2. If further medical attention is necessary after reporting to supervisor/nurse,  please go ASAP to Concentra Wesport for medical treatment.
3. If emergency treatment is necessary for serious injury or life-threatening injury, please go ASAP to DePaul Medical Center's emergency department.
4. If no further medical attention is necessary, there is no need to go to Concentra or DePaul.
5. If further medical attention is necessary, your supervisor or school nurse must contact the Human Resource Center ASAP so an Authorization for Treatment form can be sent to Concentra (or DePaul in an emergency situation) prior to your arrival at Concentra or DePaul.
6. The Human Resource Center will send an Authorization for Treatment Form to Concentra or DePaul ASAP at this point.
7. Whether further medical treatment is required or not, all employees injured on the job must complete the Employee Injury Incident Report.
8. Whether further medical treatment is required or not, all supervisors of an employee injured on the job must complete the Supervisor's Injury Incident Report.
